Phentamax, we call it the MetaboMax, is a compounded medicine that we can get through a compounding pharmacy. It is a combination of two prescription (phentermine/Topamax) medicines that come as a brand name currently, but the monthly cost is $200-300 a month. It is formulated to be a delayed release capsule, which means that you take it in the morning, and it lasts all throughout the day but leaves the system enough by night to let you sleep.
Phentermine Is a stimulant. As a stimulant, it decreases your appetite. We use it at doses ranging from 5-25 mg per day which is less than standard dosing. Possible side effects of phentermine can be jitteriness, tremors, and an increase in heart rate. At low doses, side effects are significantly lessened. Dry mouth and eyes can also occur. Most people tolerate this medication well.
This medication is in a category of its own. It can be prescribed for migraine prevention, atypical depression, and seizure prevention. At higher doses, this medication can cause visual changes, mood changes, and numbness in the fingertips. In the doses we use for weight loss, these side effects rarely happen. The effects we do see are, a slightly metallic taste that alters food tastes and the way the brain releases the “feel good hormones” to food.
Chromium is a mineral. It is called an "essential trace element" because very small amounts of chromium are necessary for human health. It is also taken by mouth for depression, Turner syndrome, polycystic ovary syndrome (PCOS), lowering "bad" cholesterol, raising "good" cholesterol, obesity, heart attack, schizophrenia, bipolar
disorder, binge eating disorder, etc. Some people take chromium by mouth for body conditioning, including weight loss, increasing muscle, and decreasing body fat.
Chromium is also taken by mouth to improve athletic performance, to increase energy, and to treat a decline in memory and thinking skills in older people that is more than what is normal for their age. Some people experience side effects such as skin irritation, headaches, dizziness, nausea, mood changes, impaired thinking, judgment, and coordination but, again, these rarely occur in patients taking the lower doses.

Phentermine:
Heart or blood vessel disease (eg, coronary artery disease), history of or. Hypertension (high blood pressure) that is not controlled, history of or. Hyperthyroidism (overactive thyroid) or. Stroke, history of—Should not be used in patients with these conditions.
Topamax:
Have ever had an allergic reaction to topiramate or any other medicine. have kidney problems, especially kidney stones. have a blood disorder called acute porphyria
Chromium Picolinate:
People with kidney disease or liver disease should be cautious about taking high amounts of chromium.
Low Dose Naltrexone:
The liver metabolizes naltrexone while the kidney handles its elimination from the body. Thus, patients with kidney and liver problems should avoid using LDN. In addition, patients who do not suffer from these conditions but develop yellow discoloration of their eyes must visit their provider promptly.
Phentermine:
Do not use phentermine if you are also using similar medicines such as any stimulant benzphetamine, diethylpropion, mazindol, phendimetrazine, Bontril®, or Didrex®. Do not use this medicine if you also take an MAO inhibitor (MAOI), such as isocarboxazid (Marplan®), phenelzine (Nardil®,), selegiline (Eldepryl®), or tranylcypromine (Parnate®), or if you have used an MAOI within the past 14 days. Using these medicines together may cause serious unwanted effects.
Using this medicine while you are pregnant can harm your unborn baby. Use an effective form of birth control to keep from getting pregnant. If you think you have become pregnant while using this medicine, tell your doctor right away.
This medicine may be habit-forming. If you think this medicine is not working properly after you have taken it for a few weeks, do not increase the dose. Instead, check with your doctor.
Topamax:
Do not drink alcohol while taking topiramate (brand examples: Topamax, Qudexy XR Trokendi XR). Dangerous side effects or increased seizures may occur. Increased sleepiness or dizziness may occur if you drink alcohol while taking topiramate.
Low Dose Naltrexone:
Low dose naltrexone blocks opioid receptors and can precipitate opioid withdrawal, so it is important that you do not take any opioid (narcotic) pain relieving drugs at the same time as low dose naltrexone. You should not use low dose naltrexone treatment if you:
Are receiving opioid (narcotic) analgesics
Are addicted to opioids
Are withdrawing from opioids or have symptoms of opioid withdrawal
Have failed a naloxone challenge test or have a positive urine screen for opioids
Have acute hepatitis but may be allowed in patients with stable or compensated cirrhosis after considering the risks versus benefits
Are allergic to or have had a hypersensitivity reaction to naltrexone, polylactide-co-glycolide (PLG), or any other diluent or inactive ingredient in the product.
Tell your doctor or other health care provider of any recent use of opioids or any history of opioid dependence before starting low dose naltrexone to avoid having an opioid withdrawal. Your doctor may require that you pass a naloxone challenge test and/or a urine screen for opioids prior to low dose naltrexone use.
If you have been taking opioids of any kind, allow at least 7 days after your last dose of short-acting opioids or 10 to 14 days after your last dose of long-acting opioids to pass before starting low dose naltrexone.
If you are on opioids long term for conditions such as chronic fatigue and do not want to stop them, then you cannot take low dose naltrexone. Low dose tramadol (50mg two to three times daily) does not appear to cause problems with low dose naltrexone, according to some sources.
Low dose naltrexone should also be stopped at least 7 days before surgery.
